This is a legal document governed by Medical Treatment Planning and Decisions Act 2016 (VIC). This document is to be used if the Adult will not sign the Advance Care Directive personally but will instead direct another person to sign on their behalf.

It is a legal document that sets out a person’s binding instructions or preferences and values in relation to the medical treatment of that person in the event that the person does not have the decision-making capacity for that treatment.
